<p><strong>Survival &amp; success in today’s economic woes and tomorrow’s uncertainty</strong></p>
<p>In the economic environment we now know as the &#8220;new normal,&#8221; traditional approaches to business success for a medical practice or dental practice need a new examination.  In fact, the very survival of a practice is now – more than ever – tied to meeting the challenge of medical practice marketing or <a title="Dental Practice Management" href="http://blog.unadvertise.com">dental practice management</a>.</p>
<p>The economic climate – and key changes in the conditions of patient populations &#8211; have put a new premium on medical practice marketing, and on the role of the medical business consultant who generates a real return on investment in marketing a physician’s practice.</p>
<p>It’s a harsh landscape.  US Bureau of Labor Statistics reports show fourteen million Americans out of work, with more than six million without a job for more than six months. The Commerce Department told us at mid-year that growth in the gross domestic product has hovered near zero in 2011. Consumer spending has remained flat, housing prices are still falling . The ability of government to generate a solution in the foreseeable future is reflected in recent down-grade of US bond ratings.  Editors from the New Yorker to the Wall Street Journal see the government’s response as somewhere between &#8220;too little, too late&#8221; and simply off target.</p>
<p>Despite a declared &#8220;recovery&#8221; begun in 2009, data that reflect real life in the US depict a bleak reality.  Census Bureau information told us that poverty last year surged to 46.2 million Americans.  Median household income —  a financial health indicator for the middle of US population — fell 2.3% to $49,445 in 2010 vs. the previous year, and 7% from 2000.</p>
<p>One million more Americans lost health insurance.  US Census data showed the continuing a decade-long erosion in coverage, with the percentage of uninsured climbing to 16.3% &#8211; the highest ever recorded. A driving factor is the elimination or reduction of health benefits by employers as healthcare costs continue to climb.  That trend has reduced the percentage of Americans who get health benefits through their jobs from a peak of 65.1% in 2000 to 55.3% last year.</p>
<p>At the same time a so-called recovery had supposedly begun, results from medical practices across the nation confirmed a different – and palpable truth. Median revenue a practice collected per full-time-equivalent physician plunged by 7.8% in 2008, according to the MGMA report on multispecialty practices &#8211; a bellwether for overall economic trends for health care. The drivers of this deterioration have since continued unabated.</p>
<p>The business results of the medical practice are under unprecedented assault – with attempts by physicians to boost productivity and control expenses just not able to keep pace with declines in collections, average gross revenue and operating costs.</p>
<p>“Personal incomes are falling, and the rates of insured in the population are declining,” says medical marketing consultant Helmut Flasch of Doctor Relations in California. “This is a time to be the most aggressive about finding, engaging, capturing and keeping the most desirable patients in a local or regional market – and that’s driven by medical practice marketing or <a title="Dental Marketing" href="http://unadvertise.com">dental marketing</a> for the dentists.”</p>
<p>&#8220;We&#8217;ve seen doctors with superb professional skills and have the most advanced technology that are still seeing declines.  Medical practices that do not aggressively market for the highest quality patients will not survive.&#8221;</p>
<p>&#8220;This is really a situation of survival of the fittest,&#8221; says Flasch.</p>
<p>If the &#8220;fit&#8221; will survive – some even prosper – what does “fit” mean in terms of medical practice management?</p>
<p>Surely, there is no substitute for excellence in diagnosis and treatment.  There’s no substitute for employing technology to its fullest. Yet, medical practices providing superb treatment and state-of-the-art equipment are also suffering on the business side.  Thus, “fitness” on the business side of medical practice management is now the focus.</p>
<p>&#8220;The key is in marketing. Businesses small or large, local or global will build success on marketing  -  telling their story, building relationships.  Consumers make physician choices based on their perception of affinity with that doctor.&#8221;</p>
<p>The fitness equation for medical practice marketing – just as with all types of fitness – requires some careful planning, commitment and application of advanced methods.  Choices must be good ones for medical marketing dollars to be effective and to produce real return on investment.</p>
<p>Flasch lists four key areas of “fitness” for medical practice management.</p>
<ul>
<li>Maximize the impact of every marketing dollar you spend.  Enlist the support of community forums and piggy-back your messages to the community&#8217;s existence resources. In this way, you are using other people’s effort and money to spread the word for you instead of only depending on your own resources of time and money which will be limited.</li>
<li>Structure marketing outreach to do more than state a message.  Turn community members into fans, and fans into advocates who recommend and endorse you. One of the best ways to do that is to give the community and especially the community leaders a reason to weave your name into the everyday communication without necessarily touching the clinical aspect, but a common community oriented topic.</li>
<li>Use the power of the press, rather than placing ads. The press will of course have no interest in promoting your practice, but will want to report news. A small suggestion here would be to hook on to already existing press about health and health-related issues, even just about common living, and make it local.</li>
<li>Utilize communication technology. The internet and the multitude of communications that take place there are multipliers of your marketing time and dollars.  Nothing levels the playing field for the not-so-cash-rich promoter like internet marketing, and when it comes to local internet marketing most local business have not yet caught on to the immense potential this medium brings to the table.</li>
</ul>

<p>Perhaps it&#8217;s time every single one of us begin to take a lot more responsibility by first asking &#8220;what can I do?&#8221; instead of &#8220;why I am not getting what I deserve to get?&#8221;</p>

<p><img class="alignleft size-medium wp-image-2372" style="border-style: initial; border-color: initial;" title="Healthcare marketing" src="http://blog.unadvertise.com/wp-content/uploads/2012/01/CNN-Money2-221x300.jpg" alt="CNN News: Doctors Going Broke" width="221" height="300" />This quiet reality, which is spreading nationwide, is claiming a wide range of casualties, including family physicians, cardiologists and oncologists.&#8221;</p>
<p>&#8220;Industry watchers say the trend is worrisome. Half of all doctors in the nation operate a private practice. So if a cash crunch forces the death of an independent practice, it robs a community of a vital health care resource.&#8221;</p>
<p>&#8220;&#8230;an oncologist with a stellar reputation in the community who hasn&#8217;t taken a salary from his private practice in over a year. He owes drug companies $1.6 million, which he wasn&#8217;t reimbursed for.</p>
<p>Dr. Neil Barth is that oncologist. He has been in the top 10% of oncologists in his region, according to U.S. News Top Doctors&#8217; ranking. Still, he is contemplating personal bankruptcy.</p>
<p>That move could shutter his 31-year-old clinical practice and force 6,000 cancer patients to look for a new doctor.&#8221;</p>
<p>&#8220;The economics of providing health care in this country need to change. It&#8217;s too expensive for doctors,&#8221; he said. &#8220;I love medicine. I will find a way to refinance my debt and not lose my home or my practice.&#8221;</p>
<p>If he does declare bankruptcy, he loses all of it and has to find a way to start over at 60. Until then, he&#8217;s turning away new patients whose care he can no longer subsidize&#8230;</p>
<p>&#8220;Dr. Mike Gorman, a family physician in Logandale, Nev., recently took out an SBA loan to keep his practice running and pay his five employees.&#8221;</p>
<p>&#8220;It is embarrassing,&#8221; he said. &#8220;Doctors don&#8217;t want to talk about being in debt.&#8221; But he&#8217;s planning a new strategy to deal with his rising business expenses and falling reimbursements.&#8221;</p>
<p>&#8220;I will see more patients, but I won&#8217;t check all of their complaints at one time,&#8221; he explained. &#8220;If I do, insurance will bundle my reimbursement into one payment.&#8221; Patients will have to make repeat visits &#8212; an arrangement that he acknowledges is &#8220;inconvenient.&#8221;</p>
<p>&#8220;This system pits doctor against patient,&#8221; he said. &#8220;But it&#8217;s the only way to beat the system and get paid.&#8221;</td>

<p>This CNN article got a whopping 10,000 people recommending it on Facebook and got 5000 comments.</p>
<p>Most people who have commented support the doctors and want to see doctors doing well. Here is one particular comment that I think summarizes the overall viewpoint:</p>
<p><em>&#8220;Take a good look around at doctors today and see if they are living a lifestyle any better than CPA&#8217;s, bankers, or business owners. My friend is a doctor and makes a six figure income, but that is not the income that he brings home after taxes and overhead. In fact he lives in a neighborhood with plumbers, college teachers, and other doctors in a comfortable but not wealthy neighborhood. </em></p>
<p><em>He drives a 15 year old truck, His income is taken up by all that is mentioned below. He must go to med school to become a doctor $200,00.00. He must run an office, pay malpractice, etc. </em></p>
<p><em>How many people with his education and work hours do you know do not make a six figure income? Should he do all of this for nothing? Is there any other profession wherein a person works like this an assumes tremendous responsibility for people&#8217;s lives and make less? </em></p>
<p><em>No one should assume that doctors should just do all of this for little income. </em></p>
<p><em>Ethics aside, medicine is a business. </em></p>
<p><em>Tell me any other profession where a person has this type of education and work hours and makes no money doing it and is happy. I really want to know.&#8221; </em></p>
<p><strong>Dear Doctors,</strong></p>
<p>If you do not realize that this fact of doctors working harder and making less is already upon ALL health care professionals, no matter how you think it would never happen to you, you will one day be caught with having to cut back drastically, having to struggle to support your kids to college and wonder why despite working so hard, you cannot build a nest egg and have to work beyond retirement age.</p>
<p>Maybe you are already at that point that despite the last 10-12 years of good economic growth, you do not have a nest egg and now with a crashing economy, that goal of having financial stability is further than ever before!</p>
<p>As Einstein has said, &#8220;<strong><em>Definition of Insanity is </em></strong><strong><em>doing the same thing over and over again and expecting different results.&#8221;</em></strong></p>
<p>So instead of doing the same things over and over again, what can you do that is DIFFERENT?</p>
<p>First of all, be honest with yourself. Answer these questions:</p>
<ul>
<li>Have you known that you need to work harder on promotional strategies to get your name in front of patients that could afford your services but you have never step on the gas pedal fully and honestly?</li>
</ul>
<ul>
<li>Are you depending only your insurance to bring you patients and depending on their timetable to pay you?</li>
</ul>
<ul>
<li>Are you depending only on your patient base or referring doctors base to bring you patients?</li>
</ul>
<ul>
<li>Are you engaging in 1 or 2 marketing campaigns like Yellow Pages or newspaper ads or just the internet and think that these are sufficient for your business to survive in a harsh economy?</li>
</ul>
<ul>
<li>Are you telling yourself marketing is not important &#8211; treating patients well is ALL you need do ( despite all evidence that this is no longer enough)?</li>
</ul>
<p>If you answer YES to any of these questions, then <strong><span style="text-decoration: underline;">congratulations to your competitors</span></strong>, as you will have less and less exposure and visibility in your community and your competitors would be more than happy to see you reaching out less to potential patients.</p>
<p>Especially in a slow economy, the savvy doctor who is taking advantage of less doctors marketing in his area will STICK OUT through his marketing efforts.</p>
